Type name to agree to the following policies: Tuition payments are final and non-refundable. Tuition payments are due by the 15th of the month. A $10 late fee is required if tuition is after the 15th of the month, New Students will pay a one-time $30 registration fee. Tuition payments and registration fees are final and non-refundable. A 3% service fee is included in online payments as well as credit or debit payments. Cash or check payments do not include this fee.

Liability Release

I, the student, or the parent or guardian of student, understand that participation in the physical activity of dance at the Fremont Ballet School has a possibility to result in injury. Now, therefore the undersigned agree as follows in consideration of said child being allowed to participate in such activities and in further consideration of the valuable physical training and development of coordination which result from such activities, the undersigned parents/ legal guardians voluntarily assume all risks of accident or injury to the person of said child and hereby release Lily Adams, Elizabeth Pietrykowski, Deb Lohr, their heirs, executors, administrators or assigns, and their designated substitute teacher from every claim, liability or demand of any kind for or on account of any personal injury sustained by said child and from and against of any claims the said parents/legal guardians may or might have on account of loss of services, medical or hospital expenses, or any other claims, debts, demands, actions or causes of action which may or might occur, either to said child or to the undersigned parents/legal guardians.

Photo Release

I grant to Fremont Ballet School, permission to use photographs of my child on the internet (i.e. Social Media, Fremont Ballet School website) and/or for promotional materials, such as brochures. I understand that any pictures used in this manner will not identify my child by name.
